Horse twitch has a rope or chain I think. They look more like a hand clapper. Noise maker.

It's called a Horse Twitch.
It is used to apply pressure /
clamp the nerve on a horse's lip. Which sedates the horse and quietens it down for the Farrier / Vet to work without getting kicked.
