Mezco Hellraiser III Hell On Earth Pinhead 12-inch Action Figure
Not quite 1/6 scale, it's about a cm shorter than Hot Toys etc but it's a good size nevertheless. I added a picture of it with 1/6 scale figures for comparison.
I have the box in storage so while the photos of the figure are accurate, the box is a photo I found online to give an indication of what it looks like.
